Working across Primary Care, Community & Workplace settings, the service supports people and their families by providing long term support to improve their health and wellbeing and reduce the incidence of long term health conditions associated with drug and alcohol use.

Hull Primary Care Addictions Service

Core Service

Those people in the service have a number of protective factors or co-morbid health conditions, which make achieving abstinence from drug and alcohol particularly challenging. So clinical recovery is primarily focused on maintaining stability.

Where individual circumstances change the team prepare and support people to become drug and alcohol free.

Each person has a named case manager who is their primary contact for agreeing their care plan, and co-ordinating joint working with other services such as housing, social care and mainstream mental and physical health.

Shared Care

We offer support to people in treatment for problematic drug and alcohol use in your own GP practice. The service offers

  • One to one support

  • Assessment and review for structured treatment

  • Advice, information, and Brief Interventions for drug and alcohol concerns

  • Working in partnership with GP’s to support people in treatment

  • Support GP’s with prescribing

  • Referral to other services, including housing support, debt management, Education and Employment, Social care, family and carers support services

  • Promoting individual recovery and supporting to maintain abstinence and prevent relapse

  • Testing for Blood Borne Viruses and referral to specialist services

Enhance Prescription Medication Service

Aiming to support people on high doses of prescribed opiates. We support people who are registered with a Hull GP practice.

Enhance aims to prevent dependence and support people to reduce or stop prescription medications that can cause dependence, where the medications are no longer required.

ASSIST

Our Pop-up Clinic provision within primary care, community & workplace settings uses a recognised screening tool to identify potentially harmful and problematic alcohol and drug use amongst the public to identify those who need support.
