Working across primary care, this service supports people and their families by providing long term support to improve their health and wellbeing and reduce the incidence of long term health conditions associated with drug and alcohol use.

Hull Primary Care Addictions Service

Core Service

Those people in the service have a number of protective factors or co-morbid health conditions, which make achieving abstinence from drug and alcohol particularly challenging. So clinical recovery is primarily focused on maintaining stability.

Where individual circumstances change the team prepare and support people to become drug and alcohol free.

Each person has a named case manager who is their primary contact for agreeing their care plan, and co-ordinating joint working with other services such as housing, social care and mainstream mental and physical health.

Prescription Medication Service (Pilot)

Based on an existing successful service the pilot focuses on Prescription Opioids. The service offers 8 sessions of psychosocial interventions alongside prescriber clinical support for the prescriber. A 12-week GP review will identify where there is a need for further support when a plan is agreed.
